Currently accepting applications for the position of Fatherhood Case Manager.

The Fatherhood Case Manager will provide home visitation, case management, and health education to male participants of The Family Tree Healthy Start Program. Services will be delivered in accordance with program policies. Responsibilities also include screening eligible families for risk factors, connecting enrolled families to a medical home, monitoring child health, and referring families to community resources in order to meet their needs. The Case Manager will operate cooperatively as part of a multi-disciplinary team. Additional duties include recruiting new participants into the program through individual and team outreach, creating new linkages with community based organizations through the case management process, engaging in weekly supervision, monthly staffing, bi-monthly case conferences and Community Action Network (CAN) meetings, and participating in training sessions as recommended by the Supervisor and Project Director.

Education and Training: High School Diploma and at least 2 years of relevant work experience in a health/social service agency is required. Bachelor’s degree in communication, public relations, or comparable human services field is preferred.

Qualifications and Experience: Experience in father involvement education, case management, maternal and child health, infant mental health and/or mental health in a non-profit setting preferred. Knowledge and proficiency in a variety of computer programs (e.g., Microsoft Word, Excel, and Adobe) and experience with database management is preferred. Must demonstrate highly developed interpersonal, verbal, written, and listening skills and be culturally sensitive to the population served. Competence in providing health education and supportive services to adults, adolescents, and children. Reliable transportation, vehicle insurance and a valid driver’s license is required
