General Information:

What services are offered at the Family Tree?

The Family Tree has a number of programs and services. We provide counseling, education, and information services. We have a Community Education Program that offers education classes on topics such as nurturing fathers, anger management for adults and teens, parenting, and divorce. We offer counseling on a sliding fee scale based on income and we also provide services to those who have insurance benefits. Our lending library is open to our clients and includes resources for individuals, parents and children. Healthy Start is a program for mothers which includes home based services from social workers, nurses, and outreach coordinators as well as education. FIMR the Fetal and Infant Mortality Review is a coalition of health care provider, nurses, doctors, and prevention specialists who meet to discuss the high infant mortality rates in the region as well as to raise awareness of the high rates of infant deaths. Louisiana Quality Rating System or QRS is a program which rates early childcare/daycare centers based on a 5 star rating scale.

Does The Family Tree provide utilities assistance?

No. The Family Tree is unable to provide utility assistance. We can offer assistance in referrals to an agency that does offer help in this area.

Does The Family Tree provide rental assistance?

No. The Family Tree is unable to provide rental assistance. We can offer assistance in referrals to an agency that does offer help in this area.

Does The Family Tree provide housing assistance?

No. The Family Tree is unable to provide housing assistance. We can offer assistance in referrals to an agency that does offer help in this area.

How do I qualify for Healthy Start?

Ways to Work:

What is Ways to Work?

Ways to work is a Family Loan Program that provides loans for Used Car Purchases & Car Repairs. Loans Range from $750-$4000

What are the qualifications for Ways to Work?

  • Applicants must reside in Lafayette, Iberia, St. Landry, St. Martin or Vermilion Parishes

  • Applicants must be employed

  • Applicants must be a parent involved in their child’s life

  • Applicants must demonstrate a willingness to repay loan

What do I need to do to get into the Ways to Work program?

Call (337)981-2180 and ask about the program. The person who you speak with will gather your information and inform you of when the next orientation is. After attending orientation you will be directed on the next steps in applying for and securing a loan.

 

Counseling:

How do I get an appointment with a counselor?

Setting up an appointment with a counselor is easy. Just print and fill out the Application for Services found on our “FORMS” page and return it to our office. If you do not have access to a printer you can come into our office and fill out the paperwork. You may return the paperwork by fax, mail, or in person.

Fees for services are determined on a sliding fee scale based on gross annual household income. A nominal one time processing fee is charged in addition to your first session fee.

Once your paperwork is completed and turned in with your processing fee, you can schedule an appointment.

My child needs counseling, is there an age limit for services?

Currently our Counseling services are provided for children age 4 and up.

I don’t have insurance, can I receive counseling?

Yes. You can receive counseling services through our sliding fee scale counseling services which are based on your gross annual household income. Sessions may be as low as $15 per session.

Do you accept insurance?

Yes. We accept most major forms of insurance.

Do you accept EAP benefits?

EAP benefits are Employee Assisted Benefits. These are not the same as insurance benefits. To find out if you have EAP benefits through your employer contact your Human Resources department.

What are EAP benefits?

Are Employee Assisted Benefits. These are not the same as insurance benefits. These are benefits given directly from your employer that cover free counseling sessions. To find out if you have EAP benefits through your employer contact your Human Resources department. 

Do you accept Medicaid/Medicare?

No. Currently we are not a Medicaid or Medicare provider.

I need a letter from my counselor what do I need to do to obtain one?

This request must be made to the counselor from which you receive services. You must sign a “consent to release information” form and ask your counselor for the letter.

What classes do you offer?

I need to take Children Cope with Divorce, is this class still offered?

Children Cope with Divorce has been renamed TransParenting. The course content is the same, but the name and procedures have changed.

I am court ordered to attend a class before my next court date, but there aren’t any available. What do I do?

Call The Family Tree and register for the next available class. You can then take your registration slip to your court date to show that you are registered for and plan to complete the next available class.

I need a copy of my certificate from a class I attended. How do I get one?

There is a $10 certificate reproduction fee for those seeking a copy of their certificate. Call the office (337)981-2180 and request a copy of your certificate. Your records will be searched, and you will then be informed of the next step
