What services are offered at the Family Tree?

The Family Tree provides the following counseling, education and information services:
  • Counseling services are provided in a professional, accessible and compassionate manner to individuals, couples, families and children.
  • The Community Education Program offers workshops on topics such as fathering, anger management, parenting and divorce.
  • The Lending Library is open to the public and includes resources for individuals, parents, families and children.
  • Healthy Start is a free case management and education program for mothers and their children.
  • Nurse-Family Partnership provides free in-home visits for at-risk pregnant women by registered nurses throughout pregnancy until the child turns two years old.
  • Acadiana Maternal-Fetal-Infant-Child Death Review is a statewide initiative is to improve maternal, fetal, infant and child mortality and morbidity rates by providing regional leadership via surveillance, monitoring and preventive programs of the Maternal Child Health Program of the Louisiana Office of Public Health.
  • Louisiana’s Quality Rating System (QRS) rates early childcare/daycare centers based on a five star rating scale.
  • Ways to Work is a loan program assisting credit-challenged parents with the purchase of a car.

Where are you located?

The Family Tree is located at 4540 Ambassador Caffery Parkway, Building D, Suite B220, Lafayette, LA 70508 on the Campus of Women’s and Children’s Hospital.

Does The Family Tree provide utilities, rental or housing assistance?

No. The Family Tree is unable to provide this type of assistance but does provide referrals to agencies that can help.

  

Counseling:

How do I get an appointment with a counselor?

Fill out an Application for Services and return it to The Family Tree office by fax, mail, e-mail or hand delivery. If you do not have access to a printer, fill out the application in person at The Family Tree office.

Once your paperwork is completed and received with a nominal one-time processing fee, you can schedule an appointment.

How old must a child be to receive counseling at The Family Tree?

Counseling services are provided for children as young as five years old.

Do you accept insurance?

Yes. The Family Tree accepts most major forms of insurance including TRICARE, the health care program for uniformed service members, retirees and their families.

Do you accept EAP (Employee Assistance Program) benefits?

Yes. EAP benefits are given directly by your employer and cover free counseling sessions. To find out if you have EAP benefits, contact your Human Resources department.

Do you accept Medicaid/Medicare?

No. The Family Tree is not a Medicaid or Medicare provider.

I don’t have insurance, can I receive counseling?

Yes. You can receive counseling services on a sliding fee scale based on your gross annual household income. Each session may be as low as $20.
 

I need a letter from my counselor what do I need to do?

You must sign a 'Consent To Release Information' form at The Family Tree office and ask your counselor for the letter.

 

Education:

I need to take Children Cope with Divorce, is this class still offered?

Children Cope with Divorce has been renamed TransParenting. The course content is the same, but the name and procedures have changed.

I am court ordered to attend a class before my next court date, but there aren’t any available. What do I do?

Call The Family Tree and register for the next available class. Take your registration slip to your court date to show that you are registered for an plan to complete the next class.

I lost my certificate from a class I attended.   How do I get a copy?

There is a $10 certificate reproduction fee for those seeking copies of their certificates. Call the office (337)981-2180 and request a copy of your certificate.
